On or about May 1, 2025, during the installation of siding material at a job site located at 1450 Broadway Ave, Bedford, OH 44146 on the south side; the employer did not ensure an employee working on a pump jack scaffold was protected from a fall ranging between approximately 10.6 to 12 feet by utilizing a fall protection system/device. b. On or about May 1, 2025, during the installation of siding material at a job site located at 1450 Broadway Ave, Bedford, OH 44146 on the west side; the employer did not ensure an employee working on a pump jack scaffold was protected from a fall ranging between approximately 11 to 12 feet by utilizing a fall protection system/device.
