29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 1774 Flat Shoals Rd SE/ Lot 108-112:  On or about 04/22/2025, the employer exposed employees to fall hazards of 21-feet 2 inches above the lower ground level, in that an adequate fall protection system was not provided to employees while they were installing flashing on a roof with a pitch of 6/12.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) 1774 Flat Shoals Rd SE/ Lot 108-112: On or about 4/22/2025, the employer exposed employees to fall hazards of than 21-feet 2 inches above the lower level, in that the ladder used to access the roof of the structurer was not extended at least three feet above the upper landing surface.
