(a) On the East side of the building - On or about May 01, 2025, the employer exposed employees to struck-by hazards, in that, protective helmets were not worn by employees that worked on the ground below framing activities above them.

(a) On the roof at the jobsite - On or about May 1, 2025, the employer exposed employees to a fall hazard, in that, fall protection was not used by employees that installed felt on the roof of the house with a 9-foot fall to ground distance.
