On or about October 2nd, 2025 and times thereafter, the employer did not institute an effective  hearing conservation program for employees working in the Punch Press Department with noise exposure of 93.2dBA and 97.8dBA which is equal to a Time Weighted Average of 92.6dB and 97.2dBA.  Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR  1910.95(d)(1): When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer shall develop and implement a monitoring program.  a) At the facility 5 Harbor Park Drive, Port Washington NY 11050. On or about October 2nd, 2025 and times thereafter, the employer did not develop and implement a monitoring program for employees working  in the Punch Press Department whose noise exposure exceeded an 8-hour TWA on 85dBA, with noise exposure of 93.2dBA and 97.8dBA which is equal to a Time Weighted Average of 92.6dB and 97.2dBA.  Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
