29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels (dBAs), the employer did not develop and implement a monitoring program: a) Pallet Dis-Assembly Area- A Quest Technology Edge 5 dosimeter (Serial # EST 70295) was placed on an employee working within the pallet processing area. The employee was exposed to noise at a level of 106 % of the allowable limit during a 118 minute sampling period with exposure calculations including a zero increment for the 30 minutes not sampled. This exposure was equivalent to a sound level of 90.5 dBA for the dose and time of the employee exposure. The employer did not develop a noise monitoring program for employees exposed to levels of noise exceeding 85 dBA.

It was observed to have a sticky substance on its floor upon entering the facility due to it's continual use and unsanitary condition.
