29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) At 15112 Packing House Road, Dade City, FL 33523: On or about 4/23/2025, the employer exposed employees to struck-by and caught-in hazards, in that, employees had not been trained and evaluated on a powered industrial truck safety and operation on each model and type used, since being hired on to the company, prior to operating a Type VII telehandler 12k 55' forklift.
