29 CFR  1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads:  a) On or about April 24, 2025, at the Foxdale Ave. Water Main Project in Winnetka, Illinois, the employer did not ensure that shields, such as the Build-A-Box Modular Trench Shield BAB-4 and the Build-A-Box Modular Trench Shield BAB-6, were installed to restrict movement in the event of sudden movement.  The boxes occupied by employees were not secured to prevent movement during an unexpected event causing lateral loads.   b) On May 2, 2025, at the Foxdale Ave. Water Main Project in Winnetka, Illinois, the employer did not ensure that shields, such as the Build-A-Box Modular Trench Shield BAB-4 and the Build-A-Box Modular Trench Shield BAB-6, were installed to restrict movement in the event of sudden movement. 29 CFR  1926.652(g)(2): Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted:   a) On or about April 24, 2025, at the Foxdale Ave. Water Main Project in Winnetka, Illinois, employees were permitted to enter an excavation and perform work where the earth material was excavated approximately four feet below the bottom of the support system, Build-A-Box BAB-4 Modular Trench Shield.  b) On or about May 1, 2025, at the Foxdale Ave. Water Main Project in Winnetka, Illinois, employees were permitted to enter an excavation and perform work where the earth material was excavated approximately three feet and nine inches below the bottom of the support system, Pro-Tec Trench Shield, Serial #35715.  c) On May 2, 2025, the Foxdale Ave. Water Main Project in Winnetka, Illinois, employees were permitted to enter an excavation and perform work where the earth material was excavated approximately seven feet six inches on the north wall, four feet six inches on the east wall, and six feet five inches on the south wall, below the bottom of the support system, Build-A-Box BAB-4 Modular Trench Shield.  d) On May 2, 2025, the Foxdale Ave. Water Main Project in Winnetka, Illinois, employees were permitted to enter an excavation and perform work where the earth material was excavated approximately four feet six below the bottom of the support system, the Build-A-Box BAB-6 Modular Trench Shield. The employer did not ensure that employees were protected from cave-in hazards.  f) On May 2, 2025, at the Foxdale Ave. Water Main Project in Winnetka, Illinois, an employee was exposed to cave-in hazards while performing work in an unprotected excavation of approximately eight feet six inches in depth in the south excavation for the setting of the manhole.  The employer did not ensure the employee was protected from cave-in hazards.  g) On or about May 2, 2025, at the Foxdale Ave. Water Main Project in Winnetka, Illinois, an employee was exposed to cave-in hazards while performing work in an unprotected excavation of approximately nine feet seven inches in depth in the east excavation for the setting of the fire hydrant.  The employer did not ensure the employee was protected from cave-in hazards. A. Lamp Concrete Contractors, Inc. was previously cited for a violation of this occupational safety and health standard 29CFR 1926.651(c)(2), which was contained in OSHA inspection number 1605465, citation number 1, item number 1 and was affirmed as a final order on October 25, 2023, with respect to a workplace located at 23rd Avenue & Roosevelt Road in Broadview, Illinois.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. The Build-A-Box BAB-4 in the east end of the excavation, was not supplied with an adequate number of pins per panel, not backfilled, not within two feet of the bottom of the excavation due to extension of all adjustable legs and missing panels, and had damage to the northeast corner post.   b) On or about April 24, 2025, at the Foxdale Ave. Water Main Project in Winnetka, employees in excavations were not provided with adequate cave-in protection as the protective system that was being utilized was not in accordance with all specifications.  The Build-A-Box BAB-6 in the middle of the excavation, was not supplied with an adequate number of pins per panel, not backfilled, not within two feet of the bottom of the excavation due to extension of all adjustable legs and missing panels, and the bottom of the corner posts were not secured to the bottom hole of the extended legs.  c) On or about May 2, 2025, at the Foxdale Ave. Water Main Project in Winnetka, employees in excavations were not provided with adequate cave-in protection as the protective system that was being utilized was not in accordance with all specifications.  The Build-A-Box BAB-4 in the east excavation, was not supplied with an adequate number of pins per panel, not backfilled, and was not within two feet of the bottom of the excavation due to extension of all adjustable legs and missing panels and the way it was positioned within the excavation.  d) On or about May 2, 2025, at the Foxdale Ave. Water Main Project in Winnetka, employees in excavations were not provided with adequate cave-in protection as the protective system that was being utilized was not in accordance with all specifications.  The Build-A-Box BAB-6 in the south end of the excavation, was not supplied with an adequate number of pins per panel, not backfilled, improperly lifted and moved, and the bottom of the corner posts were not secured to all of the extended legs.   A. Lamp Concrete Contractors, Inc. was previously cited for a violation of this occupational safety and health standard 29CFR 1926.652(c)(2)(i), which was contained in OSHA inspection number 1605465, citation number 3, item number 1 and was affirmed as a final order on October 25, 2023, with respect to a workplace located at 23rd Avenue & Roosevelt Road in Broadview, Illinois.
