29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept clear from work areas, passageways, and stairs, in and around buildings or other structures.  Location: Job site;  On or about 04/30/2025, employees were exposed to trip and fall hazards while walking and working in areas and passageways of the job site where excess amounts of construction debris, scrap lumber with protruding nails, and  electrical extension cords and wiring were allowed to accumulate and obstruct paths of travel.

29 CFR 1926.34(a): Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied.  Location: Job site;  On or about 04/30/2025, employees were allowed to work inside the multi-family building under construction at the job site which had exits and egress routes which were obstructed by building debris, scrap material, electrical extension cords or temporary wiring, or were multiple feet above the ground below and were only accessible by a job-made ladder which had 3 unevenly spaced rungs which was not secured from falling and fed into the beginnings of a deck frame which employees would need to duck under or hurdle over to egress the immediate vicinity of the building.

29 CFR 1926.56(a): While work was in progress, construction areas, ramps, runways, corridors, offices, shops or storage areas were not lighted to the minimum illumination intensities as listed in Table D-3 of 29 CFR 1926.56:(a).  Location: Job site;  On or about 04/30/2025, employees were allowed to work in construction areas, corridors, and stairwells which were not lighted to the minimum illumination intensities as listed in Table D-3 of 29 CFR 1926.56(a), and in some cases were pitch black:  a) In general construction areas around the job site where the requirement is 5 foot-candles.  b) Indoor corridors, hallways, and exit ways where the requirement is 5 foot-candles.

Location: Job site;  On or about 04/30/2025, employees were allowed to utilize a main stairway to transit the different floors and egress ways of the building under construction which, where the stairway was not equipped with a stair rail system along each unprotected side or edge.
