29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) New residential construction located on homesite #30 at Continental Drive, McDonough, GA 30252, left side of the structure: On or about 05/01/2025, the employer exposed employees to fall hazards of approximately 16-feet, 11-inches to the lower level, in that a fully planked work platform on scaffold system was not provided to employees engaged in masonry activities.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access.  a) New residential construction located on homesite #30 at Continental Drive, McDonough, GA 30252, left side of the structure: On or about 05/01/2025, the employer exposed an employee to falls up to 16-feet, 11-inches to the lower level, in that a safe means of access to the work platform on the welded tubular scaffold system was not provided.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) New residential construction located on homesite #30 at Continental Drive, McDonough, GA 30252, left side of the structure: On or about 05/01/2025, the employer exposed employees to fall hazards of approximately 16-feet, 11-inches to the lower level, in that a fall protection system was not used by employees engaged in bricklaying installation using a scaffold system.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Residential construction located on Homesite #30 at Continental Drive, McDonough, GA 30252, left side of the structure: On or about 05/01/2025, and at times prior, employees were exposed to fall hazards of approximately 18-feet to the lower ground level, in that a fall protection system was not provided to employees engaged in masonry activities.
