(b) 14 Victory Circle, Mechanicsburg, PA; 2nd floor roof, front - On or about May 2, 2025, employee(s) working on the 8:12 slope roof were not protected from fall heights of approximately 6 feet to the 1st floor roof and approximately 10 feet through the 1st floor roof trusses to the lower grade. (c) 14 Victory Circle, Mechanicsburg, PA; 1st floor roof- On or about May 2, 2025, employee(s) working on the roof were not protected from a fall of approximately 10 feet to the lower grade. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 14 Victory Circle, Mechanicsburg, PA; on the 1st floor roof - On or about May 2, 2025, the two employees working on the first floor roof at approximately 12 feet above grade were exposed to a fall when using a 14-rung ladder which did not extend 3 feet past the roof edge being accessed. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
