29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: a) On or about 04/15/2025, at mechanical room area of the site, Williamsville, NY. Sump pump hole of 18-inch diameter was not protected from tripping in or stepping into or through holes by covers. Sump pump was existing and was not any part of construction work, however, equipment room was an active work area. Employees were exposed to trip and fall hazard. NO ABATEMENT CERTIFICATION REQUIRED
