29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Residential construction located at 1200 Dawn View Drive, Locust Grove, GA 30248: On or about 05/02/2025, the employer exposed employees to struck-by hazards while hoisting shingles up to the roof, in that head protection was not provided to employees working directly below the hoisting equipment.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) Residential structure located at 1200 Dawn View Drive, Locust Grove, GA 30248, Roof: On or about 05/02/2025, the employer exposed employees to fall hazards of approximately 15-feet, 4-inches to the lower level, in that a fall protection system was not provided to employees engaged in roofing activities.  Total Quality Live Roofing LLC was previously cited for a violation of this occupation safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 1569642, citation number 1, item number 1 and was affirmed as a final order date on 03/02/2022, with respect to a workplace located at Lauren Parc Townhomes at and near 3090 Lauren Parc Rd in Decatur, GA  30032.

29 CFR 1926.552(a)(4): Hoisting ropes shall be installed in accordance with the wire rope manufacturers' recommendations.  a) Residential structure located at 1200 Dawn View Drive, Locust Grove, GA 30248: On or about 05/02/2025, the employer exposed employees to potential struck-by hazards, in that the employer did not follow the manufacturer's specifications of the TranzSporter Platform Hoist as the wire cable it was manufactured with was replaced using a rope.
