29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) 351 N East Street, YORK, PA, Warehouse: On or about May 2, 2025, employees were exposed to electrical hazards at the storage warehouse due to damaged sheathing on a flexible cord (extension cord). The flexible cord initiated at the outlet located in the ceiling rafters and extended to the floor, where it powered an arcade video game machine. Corrected during the inspection, no abatement certification or documentation required.

29 CFR 1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: a) 351 N East Street, YORK, PA, Warehouse: On or about May 2, 2025, the electrical switch box located outside of the office was missing a faceplate, exposing employees to an electrical hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
