29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production Floor - Roll form Line 7 - The employer allowed employees to perform routine servicing and maintenance (dye exchange) on the 60 Ton Rousselle Press Model #6B78 with only the emergency stop engaged to control hazardous energy and not fully utilizing control of hazardous energy procedures, thus exposing employees to amputation hazards. This condition was observed on or about May 5, 2025. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Production Floor - Roll form Line 7 - The employer allowed employees to perform routine servicing and maintenance, dye exchange, on the 60 Ton Rousselle Press Model #6B78 with no lock out or tag out devices affixed as required only the emergency stop engaged to control hazardous energy, thus exposing employees to amputation hazards. This condition was observed on or about May 5, 2025. b) Production Floor - Roll form Line 14 - The employer allowed employees to perform maintenance on Roll Form with 40 Ton Rousselle Press Model #4B with no lock out tag out devices affixed as required to control hazardous energy, thus exposing employees to amputation hazards. This condition was observed on or about May 5, 2025. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
