29 CFR 1910.1200(e)(1):Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: Employee(s) are exposed to hazardous chemicals that can cause eye and skin irritation. This was most recently documented on or about May 5, 2025, located at Emiline Street & South 70th Street, La Vista, Nebraska. Employees were exposed to the following but not limited to, Arctic 6169 PVC Medium Bod and JIM PR-1L PVC Primer when engaged in plumbing activities.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems shall be made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: Employees were exposed to engulfment hazards. On or about May 5, 2025, employee(s) engaged in residential sewer repairs at Emiline Street & South 70th Street, LaVista, Nebraska. Employee(s) were working in an excavation that was over 5 feet deep with vertical walls and had not been evaluated for hazards by a competent person on site.
