29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition On or about 5 May 2025, employees of US Polymers-Accurez, LLC, located at 300 East Primm Street in St. Louis, Missouri, were exposed to Powered Industrial Truck (PIT) hazards due to operation on work surfaces that were not maintained in a safe condition. Forklifts are operated on a sloped parking lot with damaged asphalt.
