29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) 201 S Hellertown Ave Quakertown Pa, 18951, Maintenance shop:  On or about May 6, 2025, employees used a Rigid NO 341 pipe threader which was unguarded to prevent accidental contact at the point of operation, exposing the operator/employee to an entanglement hazard.  b)  201 S Hellertown Ave Quakertown Pa, 18951, Upper Resin Blending:  On or about May 6, 2025, employees operating a Blue Hockmeyer High Speed Disperser bearing M3072 sticker,  which was unguarded to prevent accidental contact at the point of operation, rotating parts, exposing the operator/employee to an entanglement hazard.  c) 201 S Hellertown Ave, Quakertown PA, 18951, Upper Resin Blending: On or about May 6, 2025, employees operating a large white colored Hockmeyer  High Speed Disperser,  which was unguarded to prevent accidental contact at the point of operation, rotating parts, exposing the operator/employee to an entanglement hazard.  d) 201 S Hellertown Ave, Quakertown PA, 18951, Polyurethane area: On or about May 6, 2025, employees operating a blue colored Hockmeyer High Speed Disperser,  bearing sticker M3078, which was unguarded to prevent accidental contact at the point of operation, rotating parts, exposing the operator/employee to an entanglement hazard.  e)  201 S Hellertown Ave, Quakertown PA, 18951, R&D area: On or about May 6, 2025, employees operating a blue colored Hockmeyer High Speed Disperser, which was unguarded to prevent accidental contact at the point of operation, rotating parts, exposing the operator/employee to an entanglement hazard.  f) 201 S Hellertown Ave, Quakertown PA, 18951, R&D area: On or about May 6, 2025, employees operating a blue colored tabletop Hockmeyer High Speed Disperser, which was unguarded to prevent accidental contact at the point of operation, rotating parts, exposing the operator/employee to an entanglement hazard.  g) 201 S Hellertown Ave, Quakertown PA, 18951, R&D area:  On or about May 6, 2025, employees operating a tabletop Hockmeyer High Speed Disperser, with the number 9 on it, which was unguarded to prevent accidental contact at the point of operation, rotating parts, exposing the operator/employee to an entanglement hazard.  h) 201 S Hellertown Ave, Quakertown, PA, 18951, Maintenance shop: On or about May 6, 2025,  employees utilizing Shopforce Power Equipment 17' Drill Press, that is not guarded to protect employees from rotating parts, point of operation or flying debris.  i) 201 S Hellertown Ave, Quakertown, PA, 18951, Maintenance shop:  On or about May 6, 2025,  the Grizzly  horizontal bandsaw, bearing Model No. G1758 Serial number 073301, is being operated without a properly adjusted guard. Employees are exposed to unguarded portions of a horizontal band saw blade.  j) 201 S Hellertown Ave, Quakertown, PA, 189511, Maintenance shop: On or about May 6, 2025, the DeWalt portable bandsaw, serial number 038723, is being operated without a properly adjusted guard. Employees are exposed to unguarded portions of the vertical band saw blade attached to a shop made table.

29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) 201 S Hellertown Ave, Quakertown, PA 18951, Maintenance shop: On or about May 6 2025, employees were exposed to electric shock while utilizing a yellow extension cord as a permanent power source. The three pronged cord was used to power a Grizzly horizontal bandsaw, A Rigid NO 341 pipe threader and a shop made outlet/switch box that ran a DeWalt portable bandsaw which was mounted to a fabricated table.
