29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions: On or about May 2, 2025, during the installation of framing material (roof trusses) at a job site located at 83 E. Main ST, Salineville Ohio 43945; the employer allowed employees working on a baker scaffold to gain access to elevated working surface to move the scaffold while standing on the working platform approximately 8 to 9 feet above the ground. The surface that the mobile scaffold was moved on was not clear of debris, tools, and material to be installed, then when the scaffold struck the debris the scaffold tipped over.
