On or about May 6, 2025, in the northwest section of the jobsite, employees utilized a trench where the surrounding structures were endangered by excavation operations without support systems while installing power conduits.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about May 6, 2025, on the southeast end of the trench, an employee utilized a ladder that was not located within 25 feet of lateral travel while installing power conduits. Karlsruher, Inc. DBA CSA Constructors was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1757197, citation number 1, item number 2, and was affirmed as a final order on August 16, 2024, with respect to a workplace located at Loop 375 Frontage Road Between Bluff Creek St. and Resler Dr.
