29 CFR 1926.251(a)(2)(i): Employers must ensure that rigging equipment has permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load. On or about May 5, 2025, a chain without identification markings/ tags was used to hoist concrete basin cap, exposing employees to the hazards associated with using a chain with an unknow capacity. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1153(e)(1)(i): Respiratory protection was not provided where specified by Table 1 of paragraph (c) of this section: On or about May 5, 2025, employees dry cutting concrete with a handheld power saw were not wearing respiratory protection as specified in Table 1, exposing employees to the hazards associated with respirable silica. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
