29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) 49 York St Gettysburg, PA 17325 - employees were exposed to fall hazards in excess of 18 feet above grade while conducting leading edge work without utilizing any form of fall protection, on or about May 5, 2025.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) 49 York St Gettysburg, PA 17325 - first to second story ladder- employees were exposed to fall hazards to the next lowest level  while conducting leading edge work due to ladders which did not extend 3 feet above the upper landing surface, on or about May 5, 2025.  b) 49 York St Gettysburg, PA 17325 - second to third story ladder- employees were exposed to fall hazards to the next lowest level  while conducting leading edge work due to ladders which did not extend 3 feet above the upper landing surface, on or about May 5, 2025.  Abatement certification and documentation required within 10 days after abatement date.
