Jefferson Street, Naperville, IL 60540 - On or about May 7, 2025, the employer did not provide a fall hazard training program for employees thereby exposing an employee required to perform vent stack removal operations at the unprotected edge of a roof to a fall of 13.6 feet (4.14 m). Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

The employee ascended and descended the ladder, using while it was in a closed configuration and leaned against an exterior wall, in order to reach and exit an upper landing thereby being exposed to falls of 7.6 feet (2.3 m) which may result in permanently disabling injury or death. Abatement documentation is not required of this item.
