(a)Blue Grass Landscaping & Gardening LLC, 3974 US 1 North, Monmouth Junction, NJ, 08852/ The employer did not ensure that each employee using the Warrior 4-1/2" angle grinder were given guidance per the operators manual which states to not attach a toothed saw blade because such blades create frequent kickback and loss of control, this exposed employees to amputation hazards. Condition noted on or about 05/06/2025.
