The employer did not ensure that the worker completed powered industrial truck training and evaluation prior to use.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: (a) Worksite, roof: On or about April 17, 2025, employees were working along the edge of a flat roof of a school building approximately 40-feet above the ground. The guardrail system was constructed from pieces of wood and was not capable of withstanding at least 200 pounds of force exposing employee to fall hazards.
