29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Commercial flat roofing project, 701 Main Street, East Hartford, CT:   On or about May 8, 2025, the employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing work on a flat roof that was greater than 50 feet wide.  The employees were exposed to fall hazards of approximately 16 feet form the edge of the flat roof tot he ground.  All Phase Renovations, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR  1926.501(b)(10), which was contained in OSHA inspection number 1624498, citation number 1, item number 01 and was affirmed as a final order on December 15, 2022, with respect to a workplace located at 1 Worcester Road, Framingham, MA.
