29 CFR 1910.133(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) On or about May 14, 2025, the employer did not ensure employees utilizing SC-CR "B" Concrete Form Release (eye irritation) to lubricate cement forms were required to utilize eye protection, exposing employees to eye irritation hazards from chemical contact. b) On or about May 14, 2025, the employer did not ensure employees utilizing compressed air at 94-127 PSI to blow out cement debris from cement forms were required to utilize eye protection, exposing employees to eye injury hazards from flying particles.

29 CFR 1910.242(b):Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment. a) On or about May 14, 2025, the employer did not ensure compressed air used for cleaning cement chips out of cement forms was kept at a PSI less than 30 in that the air employees used to perform this task was between 94-127 PSI.

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: (i):Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment; a) On or about May 14, 2025, the employer's hazard assessment was inadequate in that it did not account for the SP-CR "B" Concrete Form Release (skin irritation) soaking into employees' shirts and causing skin irritation.
