29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) On or about May 8, 2025, at the jobsite located at N108W15563 Bel Aire Lane, Germantown, WI, the employer had not ensured the attachment point of the body harness was located in the center of the wearer's back by clipping it to the D-Ring on the harness, while performing roofing work on an existing residential home. This exposed employees to a approximately 19 feet fall.

29 CFR 1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a)On or about May 8, 2025, at the jobsite located at N108W15563 Bel Aire Lane, Germantown, WI, the employer did not ensure employees performing roofing work on a residential construction site with pneumatic nail guns were utilizing safety glasses, exposing employees to eye injuries.
