29 CFR 1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about 05/08/2025, the employer did not ensure that a competent person was making frequent and regular inspections of the job sites in that there were at least 4 employees performing roofing activities on a high-slope residential building without the utilization of a fall protection system. Employees were exposed to a fall hazard of at least 10'7" fall on a slope that was 7/12 on the highest pitch. Employees were also working on higher portion of the roof that measured 21'11".

29 CFR 1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a) On or about 5/8/2025, the employer did not ensure that employees performing roofing activities on a commercial steep roof were utilizing a fall protection system. Employees were exposed to a 21'11' foot fall on the highest point of the roof and a 10'7" fall on the lower landing.

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. a) On or about 5/8/2025, the employer did not ensure that employees utilizing a ladder to gain access to an upper landing surface on a 10'7" tall roof extended at least 3 feet above the landing exposing employees to fall hazards.
