29 CFR  1926.501(b)(13):  Residential construction.  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a)  On or about May 8, 2025, at a worksite located at 1277 Autumn Field Lane, De Pere, Wisconsin, an employee was exposed to a fall hazard of 16 feet while working from an exterior wall top plate framing without using fall protection equipment.  b) On or about May 8, 2025, at a worksite located at 1277 Autumn Field Lane, De Pere, Wisconsin, an employee was exposed to a fall hazard of 11.5 feet while working from an exterior wall top plate framing without using fall protection equipment.  The Buntin Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1769592, citation number 1, item number 1a and was affirmed as a final order on October 9, 2024, with respect to a workplace located at 1273 Autumn Field Drive, De Pere, WI  54115.

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about May 8, 2025, at a worksite located at 1277 Autumn Field Lane, De Pere, Wisconsin, the employer did not ensure that employees exposed to fall hazards of 16 feet and 11.5 feet during framing work had received training in the recognition of fall hazards and methods to mitigate those hazards prior to exposure.  The Buntin Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA inspection number 1769592, citation number 1, item number 1b and was affirmed as a final order on October 9, 2024, with respect to a workplace located at 1273 Autumn Field Drive, De Pere, WI  54115.

29 CFR  1926.602(d):  Powered industrial truck operator training.  Note: The requirements applicable to construction work under this paragraph are identical to those set forth at ��1910.178(l) of this chapter.  Reference:  29 CFR 1910.178(l)(4)(iii):  An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.   On or about May 8, 2025, at a worksite located at 1277 Autumn Field Lane, De Pere, Wisconsin, Wisconsin, the employer did not ensure the Case 686G Telehandler operator had a current performance evaluation within three years for moving construction materials on the worksite.
