29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about May 8, 2025, the employer did not ensure that the Mazak Nexus 6000 Horizontal Center's (model no. HCN-6000, serial no. 172190, company no. 2141) interlock on the Automatic Pallet Changing door was working properly. The interlock had been bypassed which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips. (b) On or about May 8, 2025, the employer did not ensure that the Mazak Nexus 6000 Horizontal Center's (model no. HCN-6000, serial no. 179716, company no. 2143) interlock on the Automatic Pallet Changing door was working properly. The interlock had been bypassed which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips. (c) On or about May 8, 2025, the employer did not ensure that the Mazak Nexus 8800-II Horizontal Center's (model no. HCN-8800II, serial no. 211755, company no. 2152) interlock on the operator door was working properly. The interlock had been bypassed which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips. (d) On or about May 8, 2025, the employer did not ensure that the Mazak Nexus 8800-II Horizontal Center's (model no. HCN-8800II, serial no. 211880, company no. 2153) tool magazine viewing windows on the back side of the machine were properly guarded. Employees stored a metal tool (which was a bent metal clothes hanger) for retrieving straps in parts, on the lip of the viewing windows. While grabbing the metal tool employees hands were placed in and around the opening which exposed employees to the internal moving parts of the machine.
