29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers. Location - Entire site/multi-floors On or about April 30, 2025, employees were permitted to walk and work in areas containing multiple unprotected floor holes located throughout the new multi-family housing units, including main passageways and corridors that were frequently accessed during work activities.
