29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a) On or about May 8, 2025, an employee was exposed to head injury while working inside of a trench without using a protective helmet.  Tangney & Sons Plumbing and Sewer Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a) which was contained in OSHA inspection number 1682060, citation number 1, item number 1, and was affirmed as a final order on September 26, 2023, with respect to a workplace located at 1010 Chestnut Ave, WILMETTE, IL 60091.  	 In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
