On or about May 3, 2025, and at times prior, located in the bakery feeds department, employees operated processed equipment Air Unit 7 North Vestibule without prior inspection and testing that resulted in corrosion of the piping causing an ammonia leak.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about May 3, 2025, and at times prior, located on the roof, employees closed an isolation valve for Air Unit 7 North Vestibule to block the flow of liquid ammonia and did not affix a lockout or tagout device to the valve.
