29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator: a.) Smitchko Tool & Die, LLC DBA Interfuse Manufacturing, located at 2065 Shawville Highway, Woodland, PA 16881, Plasma Cutter Building, on or about May, 9th, 2025, and times prior thereto: Employees voluntarily wore half mask elastomeric respirators (Miller LPR-100. P100 cartridges) during deburring operations. Employees that voluntarily used respirators were not provided with a medical evaluation prior to the use of the respirators in the workplace. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Smitchko Tool & Die, LLC DBA Interfuse Manufacturing, located at 2065 Shawville Highway, Woodland, PA 16881, Plasma Cutter Building, on or about May, 9th, 2025, and times prior thereto: Employees voluntarily wore half mask elastomeric respirators (Miller LPR-100, P100 cartridges) during deburring operations. Employees that voluntarily used respirators were not provided with the information in Appendix D to this section.
