29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  On or about May 9, 2025, the employer failed to get approval for the homemade wood platform that was on the forks of the SkyJack SJ1056TH rough terrain forklift that was being used at 2 Bradley Dr, Hudson OH 44236. Two employees were standing in the homemade wooden platform with framing materials that were being nailed on the second story level.

29 CFR  1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  On or about May  9, 2025,  the employer failed to attach the homemade wood platform to the forks of  the SkyJack SJ1056TH rough terrain forklift  being used at 2 Bradley Dr, Hudson OH 44236. Two employees were standing in the homemade wooden platform with framing materials  that were being nailed on the second story level.

29 CFR  1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:   On or about May  9, 2025,  employees were exposed to falls of approximately 12 plus  feet while on the homemade wood platform supported by the SkyJack SJ1056TH rough terrain forklift and the employer did not ensure employees were protected from falling by personal fall arrest system at the worksite located at 2 Bradley Dr, Hudson OH 44236.
