On or about May 9, 2025, at the warehouse: employees were exposed to struck by and crushing hazards, in that the the Toyota forklift was operated with a missing seat belt. b. On or about May 9, 2025, at the warehouse: employees were exposed to struck by and crushing hazards, in that the Hyster forklift was operated with a missing seat belt.

On or about May 9, 2025, at the warehouse: employees were exposed to struck by and crushing hazards in that, the the Toyota forklift was operated with damaged tires and overhead lights, missing seat belt and worn foot pedal. b. On or about May 9, 2025, at the warehouse: employees were exposed to struck by and crushing hazards in that, the Hyster forklift was operated with damaged tires and missing seat belt. c. On or about May 9, 2025, at the warehouse: employees were exposed to struck by and crushing hazards in that, the Komatsu Model FG258 (Serial #137093) forklift was operated with damaged front tires.
