29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection. On or about May 9, 2025, three employees were engaged in residential roofing activities, located at 5917 North 209th St, Elk Horn, NE 68022, were exposed to fall hazards in that the employer had employees re-roofing the roof with a pitch greater than 4:12 in heights between 13 and 25 feet without without fall protection.

29 CFR 1926.503(a)(2):The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas: On or about May 9, 2025, employees engaged in residential roofing activities, located at 5917 North 209th St, Elk Horn, NE 68022, were exposed to fall hazards between 13 and 25-feet in that the employer failed to ensure employees conducting residential roofing operations were trained in the essential aspects of fall protection equipment use and recognition of fall hazards in residential construction.
