Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) On or about?May?8,?2025, the employer failed to ensure that employees were protected by a guardrail system, safety net, or personal fall?arrest system while installing trim along an unprotected flat?roof edge. As a result, employees were exposed to a 15?foot fall hazard to the level below.

29 CFR 1926.502(d)(8): Horizontal lifelines shall be designed, installed, and used, under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two. a) On or about?May?8,?2025, the employer failed to ensure the horizontal lifeline used by employees installing metal trim was properly designed and anchored under a qualified person's supervision. As a result, employees were exposed to an uncontrolled fall of approximately?15?feet to the ground below.
