29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 8, 2025, at the location of a worksite located at 31208 Manchester Ave., Bay Village, OH: employees were exposed to a fall hazard of approximately 8 -10 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface. Two ladders were used in this manner, both on the front and rear of the building.
