29 CFR  1910.146(k)(3): The employer did not use a retrieval system or methods to facilitate non-entry rescue when an authorized entrant entered a permit space.  On or about May 5, 2025, Employees were required to enter a permit-required confined space to torch cut and employer did not ensure the retrieval system met the following requirements:   a.	Each authorized entrant used a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, above the entrant's head, or at another point which the employer can establish presents a profile small enough for the successful removal of the entrant. b.	The other end of the retrieval line was attached to a mechanical device or fixed point outside the permit space in such a manner that rescue could begin as soon as the rescuer became aware that rescue is necessary.
