On or about May 09, 2025 at the site: employer was exposed to electrical hazards, in that personal protective equipment, such as electrical gloves were not used while testing 120V live wires that power LED panel fixture that was located in the ceiling.

On or about May 09, 2025 at the site: employer was exposed to electrical hazards, in that Werner type 1 aluminum A-frame ladder P/N 127148-28 was used to access an LED panel fixture that was located in the ceiling too test the 120V live electrical wires.
