29 CFR 1910.178(m)(1):Powered industrial trucks. Truck operations. Trucks shall not be driven up to anyone standing in front of a bench or other fixed object. a) On or about May 02, 2025, the employer did not ensure that powered industrial trucks were not driven up to employees standing near fixed objects. While a truck operator was removing parts from the aisles, another employee was positioned between a racking system, a cart, and a permanent cement post at the end of the aisle, exposing the employee to struck-by and crushing hazards.

29 CFR 1910.178(n)(1):Powered industrial trucks. Traveling. All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. a) On or about May 02, 2025, the employer did not ensure that a powered industrial truck operator in the materials department kept their truck under control at all times. As a result, the truck struck an employee who was located at the end of the aisle, exposing the employee to struck-by and crushing hazards.
