On or about April 18, 2025, at 4540 Ramble Rose Cir., Apt 406 in Sanford FL: the employer failed to report to OSHA within 24 -hours of a work related in-patient hospitalization of an employee. The employer was notified on April 21, 2025 and notified OSHA on May 9, 2025 of the in-patient hospitalization.
