On or about May 05, 2025, located at 1172 5 B's Road, Zanesville, Ohio 43701, the employer failed to ensure that an employee was protected against electric shock by deenergizing the circuit or effectively guarding the energized circuit when an employee drilled into the cable tray that housed the energized 480V electrical cable.  The condition exposed the employee to electric shock/electrocution as a result.

29 CFR  1926.416(a)(3): Before work is begun the employer shall ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, is so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer shall post and maintain proper warning signs where such a circuit exists. The employer shall advise employees of the location of such lines, the hazards involved, and the protective measures to be taken.  a.  On or about May 05, 2025, located at 1172 5 B's Road, Zanesville, Ohio 43701, the employer failed to ascertain whether any part of an energized electrical power circuit running through a cable tray was located where the performance of work may bring a person or tool into contact with the circuit. An employee was exposed to electrical shock hazards while using a DeWALT Cordless Hammerdrill/Drill Driver (Model DCD996) to drill into a cable tray that contained an energized 480V electrical power circuit. The employer did not identify the area with warning signs indicating the presence of the energized 480V electrical circuit and inform the employee performing the drilling work on the aluminum cable tray of the condition to ensure protective measures were taken.
