29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about May 14, 2025, at the location of a worksite located at 1184 Warren Road, Lakewood, OH: The employer did not ensure that employees performing roofing activities on an 10/12 sloped roof were protected from fall hazards. Employees were exposed to fall hazards of 20-25 feet to the ground below while performing roofing activities. Sandu Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard, 1926.501(b)(13) , which was contained in OSHA inspection number 1689110, citation number 1, item number 1a and was affirmed as a final order on 11/07/2023, with respect to a workplace located at 2068 Georgia Drive, Westlake, Ohio.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 14, 2025, at the location of a worksite located at 1184 Warren Road, Lakewood, OH: employees were exposed to a fall hazard of approximately 22 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface. Sandu Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard, 1926.1053(b)(1) , which was contained in OSHA inspection number 1689110, citation number 1, item number 2a and was affirmed as a final order on 11/07/2023, with respect to a workplace located at 2068 Georgia Drive, Westlake, Ohio.
