29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Worksite, New Hackensack Road, Median: On or about May 12, 2025, employees were caulking a concrete roadway median and were exposed to struck-by hazards in that they were not instructed on the recognition and avoidance of a struck-by hazard while working near a Ford F350XLT Crew Cab Pickup Truck. An employee was struck by the truck and sustained serious injuries.
