29 CFR  1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:  1345 28th St., Boulder, Colorado, 80302: On or about May 14, 2025, while finishing form connections and completing concrete pours, the employer failed to ensure that employees were protected from a fall of more than 6 feet from the top of the forms to the concrete pad below by a guardrail system, safety net system, or personal fall arrest system or alternative fall protection.
