29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. Location - Roof. On or about 5/15/2025, employees were not instructed to wear the PFAS correctly while working at heights greater than 6 feet. The lifeline was attached to the wearer's side/front dee ring to keep the lifeline in front of the employees.

On or about 5/15/2025, employees were not protected from falling to a lower level while working at heights greater than 6 feet. The GNC Construction, Inc. dba G & C Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1706035, citation number 2, item number 1 and was affirmed as a final order on 7/15/2024, with respect to a workplace located at 434 Main St. Wareham, MA. The GNC Construction, Inc. dba G & C Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1657761, citation number 1, item number 1 and was affirmed as a final order on 6/20/2023, with respect to a workplace located at 2502 Cranberry Highway, Wareham, MA.
