(a) On or about May 14, 2025, at 435 W. Chestnut St, Port Washington, WI 53074, employees conducting tear off on a residential home roof with a 11:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 13 feet 10 inches from the roof eve to the ground below on the right (west) side of the home.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) On or about May 14, 2025, at 435 W. Chestnut St, Port Washington, WI 53074, employees conducting roofing activities on a 11:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.
