29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.  On or about 5/14/2024, the employer did not protect employees who were engaged in roofing operations 6ft (1.8m) or more above lower levels while at the job site located at the 15358 Veterans Memorial Parkway, in Wentzville MO. The employees were conducting roofing operations and were exposed to the hazard of falling over ten (10) feet.   Construction Masters Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1695185, citation number 1, item number 1 and was affirmed as a final order on 10/23/2023, with respect to a workplace located at 24 Horizon Ridge Ct., in St. Charles, MO.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 5/14/2024, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing at the job site located at 15358 Veterans Memorial Parkway, in Wentzville MO. The ladder the employees used to access the roof was extended approximately 1.5 feet beyond the upper landing, or edge of the roof, thus exposing employees to fall hazards.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.     On or about 5/14/2024, the employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record each employee who were engaged in roofing operations 6ft (1.8m) or more above lower levels. The employees were not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury while at the job site located at 15358 Veterans Memorial Parkway, in Wentzville MO. The employees were conducting roofing operations that exposed them to the hazard of falling from a height of greater than ten (10) feet.
